I understand that the $500 scholarships available from the UHS/PCHS Alumni Association will be awarded during the annual all-school alumni reunion on Saturday afternoon, September 1, and I agree to be present for the awards ceremony.
I also understand that the scholarship funds are to be used for the 2018-19 academic year, and that I will not be eligible for the funds if I do not enroll and attend classes at the college I have named above for the 2018-19 academic year.
