Free sports physicals for Putnam County students entering grades 7-12 will be held from May 20 - June 18, 2021. These are done by appointment only. Call 660-947-7373 to schedule your appointment. Be sure to download, print, and completely fill out the
Pre Participation Physical Evaluation Form and
MSHSAA Eligibility andAuthorization Form
and bring with you, completely filled out, to your appointment. There are paper copies available at the school. Each student must have a parent present at their appointment.
