Sports and Activities Paperwork Compliance Update

Free sports physicals for Putnam County students entering grades 7-12 were held from May 20 - June 18, 2021. These were done by appointment only. Call 660-947-7373 to schedule your appointment if you have not done so yet. Be sure to download, print, and completely fill out the

Pre Participation Physical Evaluation Form and

MSHSAA Eligibility and Authorization Form

and bring with you, completely filled out, to your appointment.

Each student must have a parent present at their appointment.

PCR-I Paperwork to Sign: Appendix A 2021-2022 

Your physical and paperwork must all be completed and turned in to your coach or athletic director before starting practices. 

MANDATORY STUDENT PAPERWORK

If your child plans to participate this fall in middle school or high school sports, important paperwork must be completed and submitted on or before the first day of practice. Remember that students must complete 14 days of practice prior to competing in any contests.

Practice begins Monday, Aug. 9, for high school fall sports and activities, including football, girls golf,  softball, band, and cheer.

Practice begins Monday, Aug. 16, for middle school fall sports and activities, including football,  softball, band, and cheer.

Please check with your child’s school regarding practice details.

Before the first day of practice:

Physicals, proof of insurance, and Appendix A (concussion agreements, bus policy, citizenship card, and drug testing forms) are due on or before the first day of practice. Appendix A and the physical/insurance form can be found on the school website. Although MSHSAA only requires physicals every 2 years,  PCR-I still requires a sports physical every year.

Putnam County R-I Schools Athletic Office and school offices have received many questions about these requirements. We hope that this clarification will assist you in completing your student’s required documentation.
