Putnam County Memorial Hospital has given the following dates for free sports physicals. They will be done in the specialty clinic. You must bring the physical forms with you and be accompanied by a parent. You do not need to make an appointment. Forms are available outside the high school and middle school offices and attached below. To expedite the process, please have your portion of the forms filled out prior to arrival.
