Parent, Player, Coach Paperwork
If you were unable to attend the parent-player-coach meeting on July 26, 2022, you will find most of the paperwork presented below or outside the high school office. All paperwork and physicals have to be completed and turned in before the first practice. It is recommended that all paperwork be turned in a few days before the first practice to allow enough time for each student's information to be verified. The first high school practice is on August 8, 2022. The first junior high practices begin on August 15, 2022.

MANDATORY STUDENT PAPERWORK
If your child plans to participate this fall in middle school or high school sports, important paperwork must be completed and submitted on or before the first day of practice. Remember that students must complete 14 days of practice prior to competing in any contests.
Practice begins Monday, Aug. 8, for high school fall sports and activities, including football, girls golf, softball, band, and cheer.

Please check with your child’s school regarding practice details.
Before the first day of practice:
Physicals, proof of insurance, and Appendix A (concussion agreements, bus policy, citizenship card, and drug testing forms) are due on or before the first day of practice. Appendix A and the physical/insurance form can be found on the school website. Although MSHSAA only requires physicals every 2 years, PCR-I requires a sports physical every year to ensure the health and safety of our students.
