Putnam County Memorial Hospital has graciously given the following dates for free sports and activities physicals. They will take place in the Specialty Clinic (1926 Oak Street, Unionville, MO).
You must bring the physical forms with you, fill them out, and be accompanied by a parent. You do not need to make an appointment.
Forms will be available outside the high school and middle school offices and are linked below.
To expedite the process, please have your portion of the forms filled out prior to arrival.
